The DUTCH Test Explained: A Clearer Picture of Your Hormones

The DUTCH test is one of the most useful tools in functional hormone assessment because it shows not just how much of each hormone you have, but how your body is processing them. For women with confusing midlife symptoms, that extra layer often explains a great deal. For example, the breakdown product of estrogen might be a molecule that is even more estrogenic than the original hormone, which could explain estrogen-dominant symptoms even when overall estrogen levels are relatively low in the blood. The DUTCH test can also be ordered together with a cortisol awakening response. This is the most sensitive assessment of our stress axis’s resilience. 

What the DUTCH test is

DUTCH stands for Dried Urine Test for Comprehensive Hormones. You collect small urine samples at home over a day, which are then analyzed for a detailed picture of your hormones and their breakdown products. If you also do the cortisol test, you will additionally take a saliva sample over the day. 

How it differs from a blood test

A single blood draw captures a single time point during a day. The DUTCH test reveals overall hormone output over a day, including how estrogen is metabolized. In addition, it shows how your cortisol rises and falls during the day and how it’s metabolized: whether it overshoots, giving you anxiety and restlessness, or is too low so you feel fatigued all day, or is just dysregulated, so you are tired during the day and then can’t sleep at night  — information a standard blood test doesn’t provide.

What it measures

It assesses estrogen, progesterone, and testosterone and their metabolites, plus the daily cortisol pattern and certain nutrient markers — a fuller view of your hormonal landscape. It can be a very helpful adjunct to HRT, so we get the full picture of how your body metabolizes the hormones we give you. It can be a great tool for reducing the risks and side effects of HRT. 

Who it helps

It’s particularly useful for persistent symptoms despite “normal” bloodwork, for understanding estrogen metabolism, stress axis evaluation, and for tailoring or troubleshooting hormone therapy.

What happens with the results

Results are interpreted alongside your symptoms and history to build a targeted plan — the test is a means to a precise strategy, not an end in itself.
